The Health Homes Program Administrator (HHPA) will provide leadership to ensure efficient operations, quality services, positive client experiences, and compliance with CMS and in concert with the organization and applicable state and federal regulations. S/he will provide management oversight on data tracking and reporting for•Provides direct management oversight on all care management technology systems
•Delivers all required reports to the Health Home leads
•Collaborates with the fiscal team to monitor monthly variances Health Homes. The HHPA in collaboration with organizational leadership will be involved in the implementation of strategic planning of health care services to facilitate the efficiency of care delivery models that support the program’s goals and objectives.


Lead Health Home Partnerships and Business Development

- Quality Assurance and Continuous Quality Improvement

- Fiscal and Data Management

- Organizational Leadership

- Other responsibilities as assigned by the C.E.O.

Required Qualifications

Master’s Degree in Social Worker with LMSW; Social Work or a human services related field with a minimum two (2) years of clinical or case management experience in a medical, long-term care or behavioral healthcare setting

•Experience working with the chronically ill, persons with HIV/AIDS, persons with a history of mental illness, substance abuse, homelessness or chemical dependence or equivalent

•Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action

•Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, Internet research, use of online calendars and other software
