SUMMARY
As part of the Enhanced
Discharge Supports for Stronger Family Permanency initiative, the
Reunification Specialist (RS) will work with families with a Permanency Goal of
Return to Parent to strengthen the reunification process and connect
families to community services and supports. The Reunification Specialist
will report to the Supervisor/Clinical Director and work collaboratively with
case planning and other Permanency Support Services staff.
JOB RESPONSIBILITIES :
+ Receive and track referrals from case planning team of
families that are on track to trial discharge and need support and connections
for reunification
+ Perform regular assessments of parent behavior change
+ Make appropriate referrals for families to support the
reunification process
+ Facilitate Evidence-Based Model workshop: Parenting
Journey
+ Mentor families by modeling appropriate interpersonal
and parenting behavior along with skills of daily living
+ Facilitate Family Time coaching techniques during
family visitations
+ Plan and host events for families whose children are
on trial discharge (i.e. Family Fun Night)
+ Establish and maintain professional relationships with
pre-existing providers already involved in families lives
+ Establish and foster relationships among local schools
and other community providers to support the families
+ Conduct field visits to families
residence/schools/service providers
+ Maintain accurate records of all interventions and involvement with families by
documenting contacts within 72 hours
+ Responsible for attending management meetings and
providing updates on youth services to the foster care management staff
+ Contribute to unit s overall efforts to enhance
permanency options for families by performing tasks in a timely manner;
providing updates on a regular basis; observing the agency s work schedule and
attendance policies; and follow all agency regulations as outlined in the
Employee Manual
+ Execute all other tasks as directed by supervisor/Clinical Director to
support the families being served
+ Complete all other tasks as
assigned by Supervisor
Education:
Master degree in Social Work or
related field. LMSW preferred.
Experience:
One year working directly with children, youth and their families in a
Foster Care or Child Welfare setting.
