Program Overview:NADAPs Project ACE (Assessment*Case Management*Employment) is an intensive, employment:focused case management program that helps more than 700 New York City residents. Staff conducts intensive client assessments and then designs and implements comprehensive, individualized service plans. By addressing housing, substance abuse, mental conditions and legal barriers, Project ACE helps clients move toward economic independence and self:sufficiency. Job Title: Housing Specialist : Brooklyn, New York (4 month contract)The Housing Specialist will be responsible for collecting documents for completion of housing applications to assist clients with moving into permanent stable housing and coordinating in the community with realtors, landlords or other housing pletes applications for permanent housing or housing subsidy for any and all services that clients are eligible for.Documents manually and electronically clients status, contacts with client and service providers, services and interventions provided, and meetings. Adheres to documentation procedures.Perform site visits to locations including the prospective permanent housing locations, treatment programs, employment site or other community based residential agencies.Collaborates with Case Manager to identify client needs and barriers to permanent housing and self:sufficiency (medical, psychiatric, alcohol/substance abuse treatment, legal, public benefits, vocational, educational, and other needs.)Develops, follows, and documents daily schedule of appointments. Informs supervisor or other management staff of changes to schedule or appointments. Maintains accurate timesheets.Collaborates with team members of other professional disciplines, including vocational counselors, social workers, treatment providers, and housing specialists, to ensure that clients are meeting their housing goals.Education : High school diploma or GED required; AssociateA?s degree preferredMinimum of three (3) yearsA? experience locating housing for individuals or families requiredKnowledge of housing resources in New York City, Brooklyn would be idealGood verbal and written communication skillsComputer literate specifically with Microsoft Office applications required Must have existing housing agents/realtor database of contactsKnowledge of NY/NY Housing Status and 2010E applicationsExperience working with clients living with substance use and behavioral health diagnosesAbility to research housing options and work with realtors/landlordsKnowledge of client skills needed to obtain housing
